For First - Timer:

1. Philippine Statistics Authority (PSA) - issued Birth Certificate;
2. PSA -   issued   Marriage   Certificate/Contract   (for   married   female applicants);
3. Transcript of Records with scanned picture, S.O. No. (unless exempted) and Remarks “For Board Examination Purposes”
4. Original and photocopy of National Bureau of Investigation (NBI) Clearance
5. Certificate of 1,000 Case Hours of Supervised Field Instruction signed by the Agency Field Supervisor (AFS) or Faculty Supervisor (in the event that the AFS is not available or has no valid PRC license), and further certified by the current Field Instruction Coordinator or current Head of the Social Work Program who must have an updated Professional Identification Card (PIC).
6. Payment: 900 – complete

For Repeaters:

1. Original and photocopy of NBI Clearance; and
2. Certificate of 1,000 Case Hours of Supervised Field Instruction signed by the Agency Field Supervisor (AFS) or Faculty Supervisor (in the event that the AFS is not available or has no valid PRC license), and further certified by the current Field Instruction Coordinator or current Head of the Social Work Program who must have an updated PIC.

1. Notarized Application Form;
2. Birth Certificate from the Philippine Statistics Authority (PSA);
3. PSA Marriage Certificate/Contract (for female married applicants);
4. College Diploma and Transcript of Records (original or authenticated, with scanned picture and remarks: “For Board Examination Purposes”);
5. Certificate of Completion of the ten (10) months of internship signed by the Dean and Registrar with school seal;
6. National Bureau of Investigation Clearance; and
7. Other documents that may be required by the Board.

For Repeaters:

• Original NBI Clearance (valid 6 months from the date of filing of the application)

For foreign nationals:

1. Notarized Application Form (Annex A);
2. Copy of the national law or international treaty, agreement or covenant to prove reciprocity for the practice of Speech-Language Pathology between the Philippines and his/her country as provided under Section 34 hereof
3. Birth Certificate;
4. Marriage Certificate/Contract (for married female applicants);
5. Copy of passport data page or any equivalent document to establish citizenship;
6. College Diploma and Transcript of Records with scanned picture;
7. Certificate of Good Moral Character to be issued by any of the following: employer, school/college/university or any competent authority acceptable to the Board; and
8. Other documents that may be required by the Board.

For degrees taken abroad, the applicant has to submit also a Certificate of Equivalency from the CHED.
